Colorado's Center for Improving Value in Healthcare (CIVHC) created an interactive report profiling 11 chronic conditions common to Coloradans. The prevalence report allows users to view the distribution of disease by age, gender, geography, payer, and time. All-Payer Claims Database (APCD) data from 2012-2015 is reported. The aim of this publication is to identify variation in disease distribution to encourage targeted intervention.
